Im not sure that anyone has yet discovered a real wormhole in space but scientists seem to think that if they do exist they must be similar to a black hole which has such a huge amount of gravity that even light cant escape from it. that being said, a wormhole would not lead you anywhere because you would be crushed by its gravity.
Other scientists seem to think that wormholes do exist and they might be a tear in space and time and yet other scientists think that they might be a connection or tunnel between two galaxies.
In the end there is no sure answer because we have not discovered one.

It would depend on the size of the wormhole. One the diameter of a donut would do the traveler serious harm. One the size of a bus wouldn't. The survivor would merely end up somewhere else.
