Amount of gore visible in the game is largely dependent on the quality of spell effects adjustable by going to the game menu and adjusting the video settings. Particle density I believe is what you are looking for. Lowering it to the minimum makes blood and gore less noticeable but it still exists. Example - some spell effects and terrain modifications feature piles of gooey red chunks with ribs sticking up out of them. In the Northrend zone Borean Tundra killing an animal turns you red and washing it off in the water leaves a pool of blood behind.
