Yes, I have a 12 gauge choke bore barrel & lug forged in one by Iver Johnsons Arms & Cycle Works. Champion is clearly listed on the shotgun and it was manufactured in Fitchburg, MASS USA.

The Champion was manufactured from 1909 until 1956. I do not know of any source to find the date of manufacture from the serial number and the company is no longer in business.

The 410 should have a smaller frame than the 12 gauge. For $100-$150, you should be able to get a whole 410 Champion.
