How nuculer disaster occur?

This is a large topic. Let's narrow it down to nuclear power stations. A nuclear power station is a complicated thing, so there's many things that could go wrong.

To take an example, there may be a problem with the cooling system, causing the increased temperature in some part of the reactor. Naturally there are safety systems in reactors, so a "disaster" would only really occur in the event of multiple system failures or perhaps erroneous human intervention. The cooling system in a reactor utilises water, which turns to steam when given enough energy. A continued increase in temperature can lead to a build up of steam, an increased pressure in the system and maybe even a steam explosion.

Wikipedia has reasonably good entries for nuclear incidents at Chernobyl, 3 mile island and K-19 amongst others.
