A single-phase transformer works with a single-phase supply, while a 3-phase transformer is used with a 3-phase supply.

A single-phase transformer has 2 wires on the primary and secondary (ignoring taps) while a 3-phase tansformer has 3 or 4 wires on the primary and secondary.

A three phase has at least 3 wires plus a possible neutral. Single phase has only 2, unless it is center tapped which would be 3. You have to measure the voltage with a voltmeter to determine the 3 wire setup. A 3 phase transformer would have equal voltages on each leg. A center tapped single phase would have higher voltage between L1 and L2 and lower from L1 to neutral or center tap and L2 to neutral or center tap.

A single phase transformer uses one primary to secondary coil. A three phase transformer uses three, one for each phase, primary to secondary coils.
