The most obvious signs are changes in appetite and activity. They may lose weight (you need to weigh your pig regularly - every day if you suspect it's ill), they may have crusties around the eyes, sneeze, cough, squeal while peeing - it depends on the illness. Any change is an indication something is wrong.
If you're not sure whether your pig is sick or healthy take it to an exotics vet to find out for certain.
Guinea pigs don't get stomach flu like humans do. However, if you are sick, it is best to do everything possible to keep your pig from getting sick too, just in case your illness is bacterial. Bacterial illness can spread to piggies. Also, if your guinea pig is having digestive problems of any kind, get him/her to a vet that knows how to treat exotics (AKA not just dogs/cats) as QUICKLY AS POSSIBLE.
if your pig has a fever it will be lethargic refuse to eat and wiil possibly not even respond to its favorite food. your pigs nose may be dry and warm take your pig to the vet. dont wait because many small animals dont live through the night when ill or injured.

The gestation period for the guinea pig (I assume this is what the question is asking) is about 60-65 days.
The heart in a guinea pig is in the body cavity between the front legs. It is not uncommon to be able to feel heartbeats in their stomach area.
