I am 45 and just noticed a white patch behind my last molar. I had both a molar and wisdom tooth pulled on that side many years ago. I'm wondering if this is scar tissue or something more sinister.?

It is possible that it could be nothing more than scar tissue or a result from minor irritation from some other source such as food impaction. However, any change in coloration of your gum of other soft tissue in your mouth should be examined by your dentist to rule out the possibility of, as you put it, something more "sinister".
