The child probably ingested the drug if they came up positive for cocaine on a drug test.
There are some medications, though rarely prescribed to children, that can cause a positive for Cocaine. If the child has recently had surgery, medicated for urination problems, suffers from a mental condition and is medicated for it. There are a few reasons that it is possible, but the younger the child the less likely. All this is possible assuming that the cocaine was ingested
For a follicle test it is extremely impossible. The hair is tested, once a false positive is given a new sample is taken and the hair is washed. The wash water is tested and the cleaned hair is tested. If the hair is positive the child ingested it. If the hair is negative and the water is positive then the child was in an environment where illegal drugs were being used.
