yes people have been known to have whats called vasospasms that occur that constrict the blood flow spastically long enough for tissue to die there causing an heart attack...some drugs can also cause this by constricting the vessels too much, like cocaine.

No; Coronary artery disease is a heart disease; or, CAD is one a disease that progresses into to Cardiomyopathy (heart disease) and MI. CAD is an aetiology of heart disease.
Coronary artery disease (CAD) is the most common type and is the leading cause of heart attacks. When you have CAD, your arteries become hard and narrow. Blood has a hard time getting to the heart, so the heart does not get all the blood it needs. CAD can lead to: Angina. Angina is chest pain or discomfort that happens when the heart does not get enough blood. Myocardial Infarction (MI) or Heart Attack Arrhythmia or Irregular Heart Rhythm Atrial Fibrillation Heart Valve Disease Congenital Heart disease Cardiomyopathy (Heart Muscle Disease) Dilated Cardiomyopathy Hypertrophic Cardiomyopathy Restrictive Cardiomyopathy Cardiomegaly (Enlarged Heart) Pericarditis Pericardial Effusion Marfan Syndrome Heart Murmurs

It is not really the blood vessel that starts a heart attack. Instead, it is the process of narrowing of the arteries that causes it. The heart has 3 main blood vessels that supply different parts of it. If any of these vessels get completely blocked by a blood clot or fat deposition, it could lead to a heart attack. Even if the branches of these vessels get blocked, it could lead to a heart attack.
