I'm not sure if this is what you mean, but there are three types of health promotion: Primary deals with keeping people from getting disease or injury such as offering free condoms or increasing traffic control near schools. Other examples are offering education on good nutrition and keeping water sources clean. Here we assume no one has the problem and we aim to keep it that way. Secondary health promotion refers to trying to keep the diseased population from spreading. AIDs or diabetic testing to know who has the disease is a good example. Here we assume some people have the problem and we want to find out who so we can try to control it. Tertiary health occurs in helping a person who has a disease or injury to minimize the problems for the individual and to teach them ways to protect others if necessary. Here we know who has the disease and we try to help the patient deal with it.

Primary health care is basic health care or care provided by Internal Medicine, Family Practice, or pediatric doctors. This is as opposed to specialty care provided by specialists like neurologists, cardiologists, etc. Primary health care is provided by your primary care doctor (those listed above). They are the everyday doctor you see when you don't feel well. They might recommend you see a specialist if they cannot adequately handle your concern.
