Once the doctor has earned their medical degree, they then apply to which ever hospital they wish to work at. However they are not always hired. Now once employed by the hospital, it is then decided what section of the hospital they will work in, by the practice they studied in med school.
To find the theme of a story or book requires the reader to delve into the work more than once. The theme is the overall feelings that the story provides.
Some of the best ways to develop the theme of a novel is to start with character development first. Get to know your characters, from simple things to more complex issues (such as hair color to personality type and family history). Once you get your characters developed and you are comfortable with them, start on setting. Where do you want the main theme of your novel to be? Where do you feel most inspired? The theme of a novel should not be forced (is it in the city, the country, a mental hospital?). Also, experiment with something you\'ve never done before (for example, write a novel based on a hippie girl searching for love, or a murder novel, maybe even a sci-fi). Once you have your characters, setting, voice (second, third, first), your theme should come easily.
