In which conditions flash-over occur while putting a LT circuit breaker in a switchgear?

Flash-over can occur:
- If you are trying to rack in a closed Circuit Breaker (in case the interlock is unavailable);
- If the rack in operation is slow enough to develop an arc between Circuit Breaker contacts and live Bus bar (it is impossible to extinguish the arc).
Can't think of other cases but these were the ones that I've heard of and actually happened during maintenance of 11 / 6.6 kV CBs.
