The top number in a blood pressure reading is the "systolic" blood pressure. This should be between 110 and 130. Once you reach 140, you are entering high blood pressure territory, but it is the sustained high readings of 150 or more that are most dangerous causing pressure to vessels in the body that can rupture and cause a hemorrhagic stroke. The bottom number in a blood pressure reading is the "diastolic" blood pressure. This should be between 55 and 70. Once you move towards 80 and above, you are moving into high blood pressure range.
Very low pressures can indicate dehydration in otherwise healthy individuals. Many older people begin to reduce their fluid intake to have better bladder control. This is unwise, as it can make them lightheaded and increase fall risks, a far worse accident than anything that can happen from the bladder!
To answer your exact question, I would say your blood pressure is fine taken in isolation. The 146 might be the result of anxiety related to being in the Dr's office, or activity such as walking into the local drugstore and standing awaiting a prescription and sticking your arm in the free blood pressure cuff. Also, repeated readings, one after another, in the same arm, can cause the results to be skewed.
The best way to check your blood pressure is to go to the Dr. and sit comfortably. Relax while the blood pressure is checked in both arms. This should be on a bare upper arm with the correct size cuff, and the correct technique. Then wait 10 minutes and check it again, then 10 minutes and again. Take the average of the three BP readings.
Aim for 130/70 or 126/64 type numbers. Lower the salt in your diet. Walk briskly every day for about 20-30 minutes. Lose weight, Quit smoking. (If any of these pertain to you...)

The systolic may be a little high. It may be better at 130 - 140 (without knowing anything about your health).
A BP of 106/80 is pretty good for a 49 year old person.

It is good, it would be excellent if it were 130 over 61. I am 70 and that is what mine is and my doctors always tell me it is excellent.
