A version of "Carnage" (with Mac Gargan, not Cletus Kasady, as Venom impersonating Spiderman) appears in the four-issue mini-series Dark Reign: The Sinister Spider-Man (June-Sept 2009).
In the movie "Spider-Man 3," Carnage did not appear as a villain because the character was not introduced in that particular film. Carnage is a separate and distinct character in the Marvel comics, and his story was not part of the plotline for that specific movie.

That depends on the host. The Toxin symbiote (slime or suit) was originally spawned from Carnage. It's first host was Patrick Mulligan, who used it to beat Venom. But then Mulligan was killed and the suit was stolen. It was then attained by Eddie Brock, who had lost the Venom slime and wanted revenge.
