Yes, Cinderkit is Cinderpelt.
StarClan let Cinderpelt relive her life, because that was not her wanted destiny. So they sent her back, letting her become a Warrior.
It was her Second chance.
She is now know as Cinderheart.
Cinderheart does not know that she is Cinderpelt.
Sometimes she has flashbacks of the old camp in the forest and from when she was a medicine cat.
And she wonders why she has a vast knowledge of medicine and herbs. ;)

No, Cinderkit and Cinderpelt are two separate characters in the Warriors series. Cinderkit eventually becomes Cinderpelt after she becomes an apprentice.
