Well, I'm not an expert in mental health, but he kills Polonius with no remorse (making jokes about the smell his corpse will give off), contemplates suicide (when he's by himself and therefore not putting on a show of madness for anyone), has no problem sending two of his childhood friends off to their deaths, and while other characters see the ghost at the beginning of the play, when it reappears, only Hamlet can see it. While Hamlet says he is going to pretend to be crazy, it seems as though there's alot of reality in there.
Ophelia was talking to herself just after Hamlet exited from his act of insanity towards her. She was saying how noble/sane his mind use to be. This quote is in Act 3 scene 1. Hope that helps.
