There are tow places to put a Ground Fault Circuit Interrupter. There is a GFCI breaker which would be installed in a breaker box and a GFCI outlet that can be installed anywhere. Most GFCI outlets allow you to connect regular outlets to the GFCI and those outlets will also be protected.

The NEC doesn't specify. Generally they are installed vertically with the ground on bottom. In the world that I work in the ground is always installed on top. This is so if a gap exists between an outlet and the plug, anything falling into the gap hits the ground first and not across the hot and neutral wires simultaneously. Expanding on this premise an outlet installed horizontally with the neutral on top is the most safe installation. But I have never seen this done.

The "hot" wire and the neutral wire both carry current (the same amount, in fact) when a load is connected to complete the circuit. The ground wire never carries current except when a fault-to-ground situation occurs. Yes, neutral and ground wires should both be at ground potential, but NO they should not be connected at the outlet.

If the GFI outlet is tripped (the outlet, not the breaker) then it is telling you there is a ground fault which must be fixed. If the GFI outlet is not tripped, and the breaker is not tripped, but it is still not providing power, then you have a loose connection or a wiring error.
It sounds like the outlet was wired by someone that didn't know what they were doing and installed the wrong size breaker ahead of the outlet. The main purpose of selecting the right size breaker to suit the wire is to prevent these conditions from happening.

It depends. It could be improperly installed. Outlets can be GFI protected by using a GFI rated breaker in the main panel box. Look for a breaker in the panel box that has a push to test button on it. Press the push to test button then plug something into the outlet to see if it still works. If the breaker turns off the power then it is GFI protected. To reset it flip the breaker to the fully of position then back to the on position.
A breaker trips when there is too much current. If you unplug everything on the circuit and the breaker still trips then you have a wiring problem or a bad breaker. From your description it is not possible to be certain if the problem started when you installed new outlet or previously. You have to describe your problem in better detail to get a good answer. When you have a circuit that trips the corresponding breaker, you need to go through each outlet on the circuit and rule it out as the problem. This can be done by pulling each outlet, from the wall and systematically remove wires from outlets while power is off and determining when problem goes away.
