Yes. Bethesda has confirmed that they are in the planning stages of "Fallout 4". Nobody is sure if this is the exact title of the game, and more information was said to be released soon.

Also Bethesda purchased the rights and now own all the rights (legally debated) of the Fallout Franchise and told reporters that they were going to DEVELOP at least 3 Fallout games (Fallout 3, Fallout:New Vegas, and Fallout 4, which is far from being released). There will probably be more Fallout games (hopefully because they are my favorite games, Fallout 3 FTW!!) and they will probably be developed by Bethesda.

It depends. After Fallout New Vegas's recent release and the announcement of Elder Scrolls 5, a Fallout 4 could be predicted to be released in 2012, at least. If that sells well, and Bethesda decides to continue the successful series, than there likely will be a Fallout 5.

From what I can discover with a bit of web searching... An actual release date hasn't been given yet. Bethesda is planning a press conference on June 14, 2015 - and they've announced that the first full reveal of the game will take place on June 18, 2015. Therefore, a release date will be sometime after that date.

It won't be out for a long time. Bethesda is going to finish Elder Scrolls 5 before even thinking about a sequel to the Fallout-franchise. There are some disagreements with the ownership of the Fallout-series too, so it'll be a while. My guess is something like 2014 maybe...
