Systolic BP should always be higher than diastolic BP.The number on the top is the systolic blood pressure and the number on the bottom is the diastolic blood pressure. The systolic BP is the pressure in the arteries right when the blood is pumped out of the heart, therefore the pressure is bigger. The diastolic BP is the pressure in the arteries after the blood has been pumped out and before the next heart contraction. At this moment, the heart is relaxed and the pressure is much lower.
The bottom number is diastolic. A diastolic of 60 or lower is a sign of hypotension. However, some people do not have hypotensive symptoms.
Systolic and diastolic pressures approaching each other is a sign of pericardial tamponade which is when fluid begins to fill the sac around the heart (pericardium). This will cause pressure to increase on the heart which will decrease the pumping power of the heart. The heart will not be able to circulate the blood effectively so the body will begin to go into shock (diaphoretic extremities, increased pulse, increased respirations). As the pressure on the heart increases, the heart is working so hard that it begins to tire and could go into arrest. Cardiac arrest, if not defibrillated, will lead to death. - Ben, NREMT-B student
