Yes the bar exam is required.

Admission to the bar is a state exam. There is not a Federal exam for normal practice, but there are some specialties, such as patent law that requires another exam.

A Bar exam is required only as a requisite to practice law in that state - I know of no other reason one would have to take a Bar exam.

What test do lawyers take?
In order to get into an American Bar Association-accredited law school, you must first take the LSAT (Law School Admission Test). Once you get your score, you can apply to law school. Once you complete law school, you have to take the Bar Examination of the state that you want to practice in. Then, you go through an admissions process, but there's no testing after the Bar. Foreign-educated lawyers will probably just have to take the Bar Exam of the state they want to practice in.

Yes. An attorney who is moving from one state to another will have to re-take the bar exam in the state they are moving to. This is due to the vast variation in laws from state to state.
