I would say so. It isn't true and correct to the best of the person's ability and they apparently had a reason to create deception therefore fraud.
In Project Management, this is called a requirements document or specifications document.
Not if there are no assets at all. A will is simply a document that transfers a persons property to other persons after death. If there are no assets to be transferred, then there is nothing for the will to do.
If the Notary is present to witness signatures - ALL the persons actually signing the document must be present.

This is not a question--it is a statement. Ask again as a question, so it is clear what you want to know.
A notary's only duty is to ascertain the identification of the individual who is to sign the document and then witness that persons signature on the document. They have no requirement to ascertain the validity or legality of whatever the document is that is being signed.
Merged documents can be very useful ! Suppose you were running a sports club of 200 people, and you wanted to send them all the same letter... You could use a merge document to print the letter out, with each persons details merged into it.
