I believe stage fright is caused by both. I had a history of family members with major stage fright and subsequently developed it myself.

Stage fright can also hit people who aren't used to performing in front of people, and with much practice and exposure to the discomfort, stage fright is likely to be reduced or disappear all together. That's what happened to me. With enough practice, I learned to be totally confident while giving speeches.

Just remember, every motivational speaker has had some type of formal training in order to make them completely confident in front of masses of people.
