If you are going the two size process it doesn't hurt to bad.
I when from a 14g to a 4g, then a 4g to 0g and it hurt like B**ch. I almost ripped my ear, I do not recommend doing that.
Go like this 20-18-16-14-12-10-8-6-4-2-0-00-7/16-1/2 and so on. The higher you go, from what I've heard, the worse it hurts.. Typically your right ear hurts more than your left also. duno why it just does.
Use tapers or pinchers.
When you're stretching it should not bleed. Listen to your body. I should a little but not a whole lot.

∙ 12y agoNot if you do it responsibly. Clean the stretch regularly and take it slow. If you go too quickly, you could get blowouts which are VERY unattractive. If you stretch to sizes above 2g, theres a high chance that they wont close up completely so that's something to consider. I definitely wouldn't say that stretching is harmful though.
