Milligram is a measure of weight. If you have less milligrams of a drug you are receiving less drug, e.g. a lower, weaker dose. (Unless you believe in homeopathy where they think less is more - but then they probably believe in fairies too...

You should NEVER downstep your fentanyl patches by CUTTING them. The way the drug is delivered, this will completely mess up how the medication is absorbed and metabolized into your system. If you're going some 50 to 25, get a prescription for the patches at the lower dose. If you're already at the lowest dose and looking to gradually step it down, do so by only uncovering half of the sticker, apply the patch and then use tape to hold down the half that is not yet exposed. You can use the same principle to step down to a quarter of a 25 patch, but never use scissors to cut the patch itself.
Everyone's different, but in general it takes several hours (especially if you're already opiate tolerant by using OxyContin) for Fentanyl to achieve a dosage equivalent to what you're taking. You want to adjust your patch attachment time so that when your OxyContin dose is going down, the patch dose is ramping up. Your doctor should have already told you this; keep in mind that initial patch doses are skewed toward the low end of the dosage chart for safety, so your initial dose might not cut it. Make sure you've got enough breakthrough meds available until you can get your patch dose correct.
