Chapters, chapter headings, verses, book titles, etc., were added by translators of the original manuscripts as study aids, readability, etc... and were sometimes somewhat "less inspired" than the ones who wrote the books, and were not always correct in these endeavors when it came to understanding what was written.
In the case of the book of Revelation... the title added by the KJV translators, "The Revelation of Saint John the Divine" is absolutely WRONG [on so many levels, in their religious piety] -- as verified by the very first words of the "inspired text":
"THE REVELATION OF JESUS CHRIST..." (Rev.1:1).
The Revelation is Jesus Christ's... not John's. John merely wrote down what Jesus conveyed to him through a vision given to him by Christ's angel:
"...He [Jesus] sent and signified it by His angel unto His servant John: who bare record of the Word of God, and of the testimony OF JESUS CHRIST [it's Jesus' revealing testimony... not John's], and of all the things that he saw." (verses 1-2)
