I am applying green card for my parents. They are concerned that they may not be able to come USA and stay for more than 6 months each year, particularly since they are elderly and not in a great health. Is there a special rule for older people holding a US green card not to come USA every year but every two years?

No- you need a valid passport too

Green Card along with a US Passport is necessary to re-enter the US. If the person has stayed out of the US for year or longer than that, then a permit to reenter the country is also a MUST. I-131 has to be filed and approved before the person leaves US. This process includes a biometric test once the application gets through. Failing to appear for the biometric session will result in the I-131 getting rejected.

If at ll the person wants to apply for citizenship, then he/she must remain in the US on a green card with 5 yrs. If the card is a conditional green card, then it will be just 2 yrs. Also they need to accumulate atleast 5 yrs of continuous residence in the US.
