Transmission filters are INSIDE the transmission. You need to remove the transmission oil pan. When you replace the filter, plan on replacing the pan gasket as well.
You can restart the transmission computer, on your 1996 Ford, by removing the transmission, computer fuse. Keep the fuse out for 10 seconds and replace it to its original slot.

There are many things that could cause this. If it shifts hard or sometimes but not others you can try replacing the fluid and filter. Make sure to replace the filter as some places will only replace the fluid and not the filter. If it is in second gear only, the transmission computer has detected a malfunction and set a trouble code.

Yes if you are replacing tranny then yes you will have to drain it out unless you want a mess im sure you dont, so drain it out just 2 plugs on it and 2 min later replace plugs it will be empty by then.
I think you should replace it. The cost in the long run to keep rebuilding all the parts that are falling apart will be more expensive than just replacing it. the first step that should be taken is to have the transmission serviced, this should include removing transmission pan, draining fluid, removing and replacing filter and pan gasket. then if the problem persists it would be best to have the vehicles computer scanned to see if any problems are present with the transmission. usually the computer will store fault codes which will help pinpoint the problem.
