The little boy is Toshio Saeki, the son of Kayako and Takeo Saeki. All three of them haunt the house. The grudge-curse was created when Takeo murdered his wife (Kayako) and son (Toshio) in a jealous rage. His wife had a crush on another man (played by Bill Pullman in the American version of the film) and wrote about it in her diary. When Takeo found the diary, he flipped out and murdered her, Toshio, and the boy's cat. She and her son hold a terrible grudge against Takeo for murdering them. Their rage (their grudge) created the curse.
