Because Carnage is a spawn from Venom- who had to appear in a movie before him. Sam Raimi said Carnage would never be featured in any of his Spider-Man films as he did not even want to use Venom- but it was planned for Carnage to be the villain in a spin-off about Venom. The solo film for Venom has been shelved now that the Spider-Man series has been rebooted- so it is unknown if Carnage is ever going to appear in a live-action film.
The first appearance of Carnage (Issue 361) was after Spider-Man had faked his death and Venom, thinking he'd won, retreated to an island to live in peace. With Carnage around Spider-Man had to drop this act and ask Venom for help. Venom has never actually killed Spider-Man.
