There is a chemical imbalance in their brains that make that see themselves as different. Most have a low self-esteem already, haven't had a very good life, or are constantly put down. Don't judge anorexics because you don't know their whole situation. It is a mental disorder, as well as physical...the mental part contributes to the chemical imbalance that makes them see themselves as fat.
it usually starts with people calling them fat. It then gets stuck in their head, and have the constant fear that name calling will come back. So they refuse to eat or, some people are born with it, because it can be genetic, but that's very rare.
People with Anorexia can also become so severely malnourished that their brain doesn't think right and that can cause them to see themselves as bigger than they really are.
People who suffer from anorexia (a physical AND a mental disorder) have a misconception in their find. They often believe that they are fat (when they usually are not), and that they must take desparate measures, thus the anorexia, in order to lose weight. Gaining weight is commonly seen as being "weak", "giving up", being ugly, contributing to low-self esteem, or not being able to be accepted by society.

Going on a diet has nothing to do with it. Anorexia is an emotional/mental illness. People afflicted with it see themselves as fat - even if they look skeletal to the rest of us, their mirror tells them they are fat. It's a terrible illness, and not easy to treat.
