No ex-president of the United States has been killed after leaving office. Theodore Roosevelt was hit by a bullet after he was President and was running for another term and was making a speech. The bullet was rather miraculously deflected and did not do him serious damage. So far as I know, that was the only assassination attempt against an ex-president.
No, not exclusively. But campaigning is a part of what politicians do: like all other presidents, Obama has to campaign if he wants to be re-elected. The majority of a president's duties involves leading the country, making important strategic decisions about issues like the economy and foreign policy. But the rest of being president is about persuading people (voters) to keep you in office. So, while Obama does not spend all of his time campaigning, he does spend some of it visiting various cities, giving speeches and trying to gather votes-- which is exactly what Bush, Clinton, and Reagan also had to do.
