It depends on the age of the gymnast, the comfort of the gymnast, and whether it is a competition. If it is a competition and the gymnast needs underwear, there are special skin colored ones because any underwear showing can be cause for a .10 deduction (same goes for bra straps showing). If the gymnast is comfortable, underwear is really not necessary until puberty hits.

You don't HAVE to. It's not required in the USAG / FIG rules. However, for any young lady over a certain age, it's recommended to maintain modesty (not to show pubic hair or labia). Most US elite and international competitors wear one of the following:
1. a full "liner", like another leotard. Usually black, white or flesh-tone.
2. briefs, cut like the bottom half of the leotard. Usually black, white, flesh-tone, or matching the main color or the leotard. Many leotard manufacturers make panties like this (e.g. GK Elite, Alpha Factor).
3. thong or "g-string" panties. These avoid the problem of underwear showing (which can be a points deduction).
