It means that the shoulder of the road is soft and non-compacted, probably not hard-surfaced, and you could easily get stuck on the shoulder. In a situation like this, if you need to stop at the side of the road, make sure that your left wheels are still on the pavement.
The soft shoulder is a part of a road or highway that is unpaved and is usually covered in gavel. The soft shoulder sign informs drivers of the soft, unpaved ground along the edge of the road or highway.

By the fact that you are saying that the motor makes noise, I am assum ing this is a power seat belt, like on an Escort? If that is the case, then the fact that the motor is noisy indicates that the plastic gear tape, that runs the legth of the track is probably stipped, and so the shoulder belt is stuck wherever it is, and it's not going anywhere. You fix it, by replacing the assembly.

Shoulder Dystocia can lead to more severe injuries such as Erb's Palsy (temporary to permanent damage to the bundle of nerves that control the shoulder, arm, wrist and hand muscles).
