The only fix is to replace the filter housing.
The heater will be in the block behind the oil filter. The cord was optional. You generally have to buy it from the dealer.

Mine does not have a block heater. It has a oil heating element on the top of the oil filter housing. It's a '99 F350 7.3l diesel.
On my 93, it looks to be fastened to the inner fender on the passenger side hiding under the Heater blower motor and Air filter.

Visually it is more appealing to have only one heater. A four foot tank isn't too terribly large, so as long as the heater is placed in an area where the water flows around it, then one heater should be sufficient. Ideally, you could use an in-line heater, if you have a canister filter.
It's under the fuel filter inside of the fuel filter housing. It also blows out fuse number 22 when the heating element shorts out.
