Any army trying to enter the highest part of the castle (the keep) first has to pass through the most of the castle while being shot at by archers on the roof of the keep and when they are at the doors rocks and boiling oil/water will be dropped on them and other tactics will be employed to stop them entering. Once they do enter they will have to climb a spiralling staircase which, other than exhausting them, is a tactical disadvantage as anyone they meet on the stairway will have higher ground. The keep may also have a secret passage through which he may escape if it gets a bit hairy. Plain and simple, really. The highest part is always the safest part.
