I think you need the PUK code. I got mine from going to the O2 website (I'm on O2) then theres a box where you type your number and it gives you a code to unlock the phone.

You can receive a card replicator by entering other cards to your online collection. Afterwards you can use the replicators to find other cards you need to complete your collection. Some cards need 1 replicator to receive, when others need 3 to unlock. My advice is to unlock cards that use 3 replicators.
