Yes, Harry Potter is the sixth Horcrux made by Lord Voldemort. It was also the sixth Horcrux to be destroyed. Voldemort himself destroyed it.

It was created when the killing curse aimed at one year old Harry Potter backfired on Voldemort. Having just killed Lily and James his soul was split and very fragile. When Voldemort was hit with the rebounding curse his soul shattered and a part of it attached itself to the only living thing in the house: Harry Potter.

The pain in Harry's scar when Voldemort is near is caused by the Horcrux trying to escape and rejoin Voldemort's soul.

The only person who knew that Harry was a Horcrux was Albus Dumbledore who guessed and had his suspicions confirmed when Harry was revealed to have some of Voldemort's powers. Dumbledore told Severus Snape that when the time was right he had to tell Harry that he must die.

Dumbledore also guessed that Harry wouldn't actually die and that just the Horcrux inside him would which is what happened.

Voldemort didn't know Harry was a Horcrux until after he had destroyed it.
