It depends what the original offence was. Most offences have a 'time limit' on a person's criminal record. For example - a friend of mine was arrested 13 years ago who is now 54. When they checked his history for previous convictions, it failed to show an arrest that happened when he was 19.

As stated above unless the CRB check is classified as an "enhanced CRB Check" - These are often used for sensitive work roles
You could be fined or arrested.
Cautions for offences on the 'DBS List of offences that will never be filtered' will show for life.Cautions not on that list will stop showing up after 6 years have passed (2 years if you were under 18 when cautioned).
Breaking the law is a bad thing. If a misdemeanor crime is committed when the person is over the age of 18, and within the past 7-10 years, it will show up on a background check.
Criminal Records Bureau (CRB) checks were replaced by Disclosure and Barring Service (DBS) checks in 2012. Cautions for offences that are on the 'DBS list of offences that will never be filtered' will show up forever.Caution not on that list will stop showing once 6 years have passed (2 years if you were under 18 at the time of caution).
Yes.

a warrant would be placed for their arrest... and I believe they are fined...
You may be arrested and/or fined. Or nothing. It depends on the reason for which you were subpoenaed, and how important it is to the court that you be there.
