No, Jefferson's original draft of the Declaration of Independence was not accepted without change. It went through revisions and edits by a committee of five members, including John Adams and Benjamin Franklin. The committee made changes to the wording and structure of the document before presenting it to the Second Continental Congress for approval.
The original name on the first written and printed version was "In Congress, July4, 1776 A DECLARATION by the REPRESENTATIVES of the UNITED STATES of AMERICA in GENERAL CONGRESS ASSEMBLED" Then Congress ordered it engrossed and signed by the members of Congress. The engrossed version was titled: "IN CONGRESS, July 4, 1776. The unanimous Declaration of the thirteen united States of America" The engrossed version used the lower case "united" by mistake and without Congressional approval.
Making it appealing, even to those loyal to England and to also make it completely loophole-proof.

They were closely and in constant tandum with each other. It was a very successful working relationship, because they produced a working document that works and functions the same way up to our present day.
It was ratified Aug 2 1776 in Philadelphia but it was not written there. It the draft was written in the Old State House in Massachusetts by Thomas Jefferson. It was then brought to Philadelphia while the approval date indicates the 4th of July the actual signing did not take place until changes and corrections could be made to that draft. Many copies were made to be released around the colonies however the original draft has been lost.
