Like most gear, there are a few sources:
- Auction House (for green items, BoE blues and crafted items)
- Craftables (Items sets from 80-85, including a few epics)
- Random world drops (anything from greens to epic cloaks and leg items)
- Justice Points (Vendor in Stormwind/Orgrimmar for blue quality items)
- Valor Points (Vendor in Stormwind/Orgrimmar for epic quality items)
- Normal and Heroic dungeons (item level 333 in normals, 346 in heroics)
- Raiding (item level 359 epic items at the current starter raids)

First and foremost, to become a Worgen, you need to have an account that is paid up to Cataclysm. If you did not purchase the Cataclysm expansion, then you can not be a Worgen.

Depends on what kind of addons they are. You don't need Cataclysm for alot of them, though.

No, Cataclysm is currently still in Beta, although some aspects such as the UI have been released to the PTR for testing. This will be part of patch 4.0, before Cataclysm.
