Gates (they make timing belts among other items) recommends changing the timing belt on the 2.3 liter engine in your 1991 Ranger at 60,000 mile intervals if Ford doesn't have a specific recommendation . P.S. I had mine changed at 67,000 miles on my 2.3 L , but I'm not a mechanic / technician and didn't see what my old belt looked like , it was suggested that I have it done by the service writer. (salary/commission)
I believe the 2.3 L - DOHC engine in the 2004 Ford Ranger has a timing CHAIN instead of a timing belt , the 3.0 L and 4.0 L use timing chains

Only the 4 cylinders have a belt. The belt should be replaced at 60,000 miles.
