The direct hormone that causes the symptoms is Cortisol produced by the adrenal gland. But the indirect hormone that increases the production of Cortisol is ACTH produced by the pituitary gland.

Exogenous Cushing's is acquired through continued use of glucocoticoid medications.

Endogenous Cushing's is caused by a tumour in the adrenal gland causing aborrant release of corticosteroids. It could also be a pituitary tumour releasing excessive ACTH which then acts on the adrenal cortex causing cortisol release. It is also possible that it's an ectopic tumour in another location (usually lung) that is secreting ACTH. There is also a rare familial form where gene mutations are inherited that increase the susceptibility to these tumours forming.
