The nutrients that are absorbed through the wall of the small intestine get absorbed by the capillary network within the villi and they will come together to form the hepatic portal vein that goes directly to the liver for processing.

Capillaries are basically tiny blood vessels. There are many in your lung's alveoli so the blood can go through gas exchange. Gas exchange being the process of which deoxygenated blood switches the waste (Co2) is has extracted from its last circulation for oxygen from the alveolus, the then oxygenated blood heads to the heart from the lungs to be pumped out from the Aorta and arteries. Capillaries are also found in your muscles so when oxygenated blood reaches those muscles the blood has a route of which it can transfer the oxygen into the muscle tissue and extract the waste. Then it goes back into the heart as deoxygenated blood and is pumped into the lungs once again and thus continues circulation. :)
