In the olden days women would whear layers of clothes under their dresses. These clothes were called 'under cloths'. Nowadays they could be compared to what we no to be a slip. Although most of the under cloths went to the ankle, unlike most slip, and under cloths would have an elastic band at the bottom of the sleeve that would would make the sleeve puff out from about the point of the shoulder to the point of the elastic bad.
