The nuclear particles in the reactor starting going too fast. This led to everyone panicking including the man in charge of controlling the chamber. He then rammed the control rods in too fast so it 'stopped' the reaction but he then pulled them out again far too fast (they should always be pulled out slowly) because ramming them in didn't work too well. This led to everything reacting and reacting until nuclear stuff leaked and people got scared and ran away.
