The signalman actually saw his own impending death, represented by three phantoms warning him, 'Below there! Look out! Look out! For God's sake, clear the way!' He is unaware of this at the time, but knows that they are some kind of spirit.

I think that the signalman saw a spectator which kept on appearing in the tunnel and warning him, showing his upcoming death by getting railed over by the train. He usually kept on hearing voices like ,'helloa Below there! Look out!Look! For GOD's sake clear the way!
