I assume this question means you were offered a promotion and refused it.... how it's interpreted varies by the personality of your boss, but generally, (unless you give a very good explanation of a different promotion you have as a goal in the company) you have committed job politics suicide: you'll be parked where you are, the perception will be that you won't do what's asked, management will view you as a prima Donna etc.
