There are two: the incus and the talus (or four if you count two on each side of the body).
The incus is a middle ear bone. The other two middle ear bones, the stapes and malleus, have attachments with the Stapedius and Tensor Tympani muscles respectively.
The Talus, which is in the foot, also has no muscle attachments.
Cardiac muscle isn't, as its the heart. not too sure bout smooth, Google it. sure you will find the answer.
