I assume you're talking about skill books and all of them are obtainable no matter what your karma is.
In Fallout 3 you can have either Very Good Karma (750-1000, Good Karma (749- 250, Neutral Karma (249- -249, Bad Karma (-250- -749) or Very Bad Karma (-750- -1000). So Bad Karma would be the second worst Karma. If you mean "What Is The Second Worst Karma Rank" as in the title for very bad Karma @ lvl 29; then it is Demon Spawn.

Any level. You find them around. Depends on what fallout game you are playing. Some of the followers have karma requirements.
There are 8 permanent companions. Jericho and Clover if you have evil karma levels. Sergant RL-3 and Butch (the same guy from Vault 101) if you have neutral karma levels, Star Paladin Cross and Fawkes of you have Good karma levels, and Charon and Dogmeat regardless of karma level.
If you don't mind bad karma you can kill Kimba in Big Town.
You can't, but the karma loss is so minimal for one single item, it is not worth worrying about.

If they are called Regulators it is because you have bad karma and people want to kill you because you steal and stuff. Just get a better karma or get a good follower to help you. or if you have good karma get badder karma or kick their asses
In fallout 3 Sergeant RL-3 is good and will follow you if u have neutral Karma... Liberty Prime is a Big robot that destroyers the Enclave in 'Take it back'.
Bad Karma! if you have bad Karma you have to find a way to have good karma or you dont survive. if you see a Red & white triangle in front of the place where you put the code, you have to get better karma have to be a godd karma person to go to broken steel
