Speaking pseudoscientifically as a Nurse and a smoker, the risks are relatively low. Hookah smoking involved heating the tobacco indirectly via charcoal on top of a metal surface which covers the tobacco mixture. Therefore, you are basically vaporizing the nicotine and what you exhale is moisture, flavoring, and the vaporized tobacco alkaloids. That's partially why you can take some massive pulls on a hookah and create a mushroom cloud with your exhalation without burning the heck out of your throat and lungs. The filtation process through the water should also help to catch and coarse particulate matter that's coming through or being combusted.

On the otherhand.... 1. Nicotine is a potent vasoconstrictor and stimulant. This means it reduces your peripheral and central circulation while increasing your heart rate and workload. These lead to complications like cool extremeties and an increased risk of MI, HTN, and stoke, etc.

2. The ingredients - who the hell knows what is in hookah tobacco unless you package it yourself. Could be all sorts of funny preservatives one wouldn't want to smoke or ingest.
